患者は 2010年11月22日から 同病院 ICUで治療を受けている。受診時、患者は激しい痛みを自覚し、内科専門医によると、鳥インフルエンザの患者に特徴的な諸症状を呈していた。…
現在、患者は徐々に改善しており、人工呼吸器の使用を終えた。熱はなく、意識は清明で、咳のみ続いている。呼吸が正常となれば退院予定である。

There, wild birds are exposed to domestic poultry, and since those areas have had H5N1 outbreaks, the virus may spread between domestic and wild birds, according to the FAO.
渡り鳥の飛路 Central Asian Flyway (CAF)[中央アジア インド フライウェイ Central Asian-Indian Flyway, Central Asian-South Asian Flyway] の東部地域の野生の水鳥は Qinghai-Tibetan plateau 青海チベット高原 (青蔵高原)を超えて、北方および東方へ行くので、実際、毎年春に HPAI [highly pathogenic avian influenza] 高病原性鳥インフルエンザ H5N1 ウイルスをモンゴル国に拡大させる手助けになっている可能性がある。
A chart in the FAO newsletter shows that Mongolia reported its 1st H5N1 outbreak in August 2005 and its latest one in May of this year (2010). Reports from the World Organization for Animal Health (OIE) say that Mongolia also reported H5N1 outbreaks in 2009 and 2006. All outbreaks reported there have been in wild birds.
An FAO fact sheet on wildlife and H5N1 says that in 2009 large numbers of birds died at Qinghai Lake after the spring migration northward from India and Bangladesh and that more wild birds died weeks later at the northern end of the flyway in Mongolia and Russia, which “strongly supports the spread of the virus from infected wild birds.” However, the fact sheet says the overall global pattern of avian flu spread since 2005 “suggests that wild birds are not the primary spreader of H5N1 HPAI but that human movement of virus through trade [貿易], marketing [マーケティング], and fomites [感染の媒介物] likely accounts for the significant spread of the virus.”

59歳女性。2010年11月2日発症。2010年11月14日入院(病院)。肺炎と診断された。現在、危篤状態。
本症例は [中国本土] Shanghai 上海, Nanjing 江蘇省南京市, Hangzhou 浙江(せっこう)省杭州市 への旅行歴がある。発症前の数日間、生きている鳥類や家禽との接触歴はないと彼女は語った。彼女との濃厚接触者は 監視下にあり、隔離状態である。現在まで全ての濃厚接触者は無症状のままである。彼らの鼻咽頭吸引物は H5検査陰性であった。患者の感染源の調査実施中。
現在までのエビデンスから、2次[感染]拡大のない human influenza A(H5N1) infection ヒト A型インフルエンザウイルス H5N1 感染症の a sporadic case 散発症例 1名と示唆される。
現在までに中国 香港特別行政区で登録された A型インフルエンザウイルス H5N1 感染症の ヒト症例は計 21人 (1997年 18人、2003年 2人、2010年 本症例 1人)である。

The development and genetic diversity of H5N1 influenza virus in China, 1996–2006
The Hong Kong/156/97 (HK/97)-like virus that was responsible for the Hong Kong ‘bird flu’ incident was a reassortant that derived its hemagglutinin (HA) gene from Gs/GD-like viruses and the other genes from H9N2 or H6N1 viruses found in quail and other game birds sold in live bird markets (Guan et al., 1999, Hoffman et al., 2000, Subbarao et al., 1998, Xu et al., 1999).

Isolation of Avian Influenza A(H5N1) Viruses from Humans — Hong Kong, May-December 1997
Patient 1. On May 9, 1997, a previously healthy 3-year-old boy developed fever, sore throat, and cough. The child’s symptoms persisted, and on May 15, he was hospitalized. His illness progressed, and on May 18, he was admitted to the pediatric intensive care unit (ICU). On May 21, the child died from acute respiratory distress secondary to viral pneumonia. Influenza A(H5N1) virus was isolated from a tracheal aspirate collected on May 19. The child may have been exposed to ill chickens before he became ill.

Case control studies for human cases of HPAI have been performed in a number of locations, including Hong Kong SAR, Indonesia, Thailand and Viet Nam. The case control study in Thailand suggested that contact with inexplicably [どういうわけか] dead poultry was the strongest risk factor (Areechokchai et al, 2006). This differed from the situation in Hong Kong SAR in 1997 where a visit to a market selling live poultry was found to be a significant risk factor (Mounts et al, 1999), although it is now known that the live-poultry markets in Hong Kong were heavily infected at that time.

The press reported today, Aug.20 1997, that a 3-year-old boy died after contracting an influenza strain that has never been seen in humans. A specimen from the boy’s trachea was identified as “influenza A of H4N1 strain”, which is known to be found mainly in birds.

Unfortunately, the press seems to have gotten the facts mixed up a bit. The isolate was identified as H5N1 subtype not H4N1 as stated. The boy was from mainland China, but I don’t know the specific location. The virus was isolated in a human virus laboratory in China.

北海道大学が行ったサーベイランスにより野生のカモの糞から鳥インフルエンザ H5N1 ウイルスの 1株が検出された (Ministry of Environment 環境省発表)。
場所 Lake Onuma, Wakkanai City, Hokkaido (E141.7593, N45.3788, the centre of the lake not the sampling site)
大沼, 北海道 稚内市街から東 6km, 「近年 コハクチョウが呼び寄せられ、日本でも有数の飛来地となった。オオハクチョウ・コブハクチョウ、マガン、アオサギなども飛来し、冬にシベリアからウトナイ湖・クッチャロ湖へ向かう中継地として羽を休む姿が見かけられる。…Wikipedia 」
で 2010年10月14日採集 (沼の中央では採集なし) した 183検体のうち 2検体からウイルス分離。
北海道庁は 10Km以内の農場 (施設数 1)を調査したが、異常はなかった。大沼の地域で野鳥の死は観察されていない。
